Even before entering Freeburg Care Center, our Social Service Director begins working to ensure that residents are comfortable in their new environment. The Social Service Director will meet with families and residents to help provide the necessary information so that informed decisions can be made. Such decisions are never easy, but our goal is to assist you in making the best decisions possible for you or your loved one. We will also educate you about services that are available to you.
If a resident anticipates returning home, we will assist with discharge planning, including home health services and any needed equipment.
Our main priority desire is to ensure a smooth return home.
